When students with disabilities register at their college’s disability services (DS) office, they may have an intake appointment. In this meeting, a DS staff member may ask students about their past use of accommodations, ask what college accommodations they’re requesting now and why they need them, or inquire about other topics.

In an ideal situation, over the course of their high school years, students would regularly review with their case manager the accommodations they’re receiving and develop the self-knowledge they’d need to answer these questions. However, this isn’t the case everywhere.

I created this tool for my first book to be used by case managers, special educators, and parents as a way of gauging student’s knowledge and targeting areas where they need more information. I now share it as a free free resource students can complete online and print or have the answers sent to them.
